In-House Membership Plan

Guided Smiles Prosthodontics & Implant Center’s in-house membership plans are not insurance plans. They provide a way for our patients to obtain the dental care they need at more affordable fees. These plans are standalone coverage and are not to be combined or mixed with insurance benefits and they cannot be used in a worker’s compensation treatment plan. The in-house plans are for use towards treatments that fall within our area of expertise. We do not offer refunds for paid plans, even if the plan benefits were not utilized during the qualifying period. Our in-house plans cannot be applied for treatments or services rendered by another dentist outside of our treatment facility, even if the patient was referred by us.

If a patient fails to schedule their 2 cleanings during the qualifying period, this service will be forfeited. There is no roll-over period. If the patient does not show for their scheduled cleaning appointment and does not provide 24-hour notice, this service will be forfeited. Please note 24-hour notice is required to cancel or reschedule appointments.
